    We are currently seeking a LPN/Staff Supervisor in our Assisted Living area to assist with providing nursing assessments, patient care and addresses medical and emotional needs of all Capitol Lakes Terraces residents.  This position will also supervise, train and mentor Resident Assistant and CNA staff.



    Work Schedule:  11:00 a.m.-7:00 p.m. (shift may vary slightly).  Days will vary and includes every other weekend as assigned.


    • Provides nursing care according to physician's orders and/or recognized nursing standards and procedures.
    • Assist with assessments related to function and physical status of a resident upon admission, during changes of condition, medication management for the purpose of determining ability for self-administration, potential restraints, etc
    • Observes and reports symptoms and conditions of residents.
    • Assist with oversight of individualized service plan (ISP) implementation by care staff; Updating the ISP as conditions change and recognition of deviation from plan performed as needed
    • Medication management: oversight of medication storage, administration, documentation, treatments and delegated tasks
    • Obtains and writes physician orders as applicable.
    • Motivate employees through mentoring, training and encouraging teamwork. Provide constructive feedback when identifying learning needs related to clinical performance, resident care and services.
    • Assist with the training of all employees in accordance with Capitol Lakes policy/procedures and state/federal regulations by contributing to the clinical development of staff
    • Provide delegation training as required per DHS 83 for all employees trained in accordance with DHS 83, in medication administration. Responsible to maintain appropriate delegation documentation and oversight of tasks performed.
    • Maintains records reflecting resident's condition, medication, and treatments.
    • Assists in maintaining a physical, social and psychological environment which will be conducive to the best interest and welfare of residents.
    • Records oral/written report for oncoming shift for continuity of care of residents. Reviews reports from previous shifts to determine changes in status of residents.
    • Assist with ensuring compliance for state and federal regulations
    • Takes and records vitals (temperatures, respiration, pulse, and blood pressurs).
    • Recognizes and interprets symptoms, reporting patient's condition, and assisting with instituting remedial measures for adverse develop­ments.
    • Maintains accurate and complete records of nursing observations, care and reporting resident’s conditions orally and in writing, studies trends and developments in general nursing practices and techniques and evaluates their adaptability to specific nursing duties.
    • Respons­ible for working with the Health Center staff in transitioning residents amongst facilities for the appropriate level of care.
    • Reviews residents for participation in Home Health programs. Institutes referrals as needed.
    • Makes apartment visits to assess resident’s condition and takes appropriate actions.
    • Responds to emergency calls from residents and families in a timely manner.
    • Administers first aid or activates emergency medical systems when needed.Observes universal precautions in providing patient care.
    • Manages the infection control program for residents and staff members
    • Provides employee TB testing and TB reads as needed/scheduled. 



    • Current LPN license in the state of Wisconsin; current CPR certification.
    • A minimum of 1 year of experience in a long-term care environment preferred.
    • Prior experience as a LPN in a memory care, assisted living, or skilled nursing setting preferred.
    • Supervisor experience preferred but not required.
    • Must be familiar with organization, functions, policies, regulations and procedures of a Medical Clinic/skilled nursing facility as they relate to nursing services.
    • Must possess excellent communication skills and be able to work as part of a team.
    • Must maintain current knowledge of computer systems.
