    Responsibilities

    • First thing in the morning read through 24 hr report to find out what happened last night
    • Make rounds in the station
      • Check bathrooms – make sure garbage is empty, rooms not cluttered, clear pathway for resident
      • Call lights within reach
      • For those with O2 – signs on door, tubing labeled within the last week, all tubing in bags if not being used, filter was cleaned within the last week
      • Tube feeding – Syringe, tubing, and bag of formula all within the last 24 hours
      • IVs – tubing dated, IV bag has date, time, and nurse initials,
      • Resident’s name outside the rooms are correct
      • Empty beds are ready for an admission
    • Attend stand up meeting daily and discuss change in condition, T.O.s , 24 hr book, review new admits
    • Run, audit, and follow up with nurses to ensure completeness and verification – New orders, no BM, missed entries, baseline care plans, comprehensive care plans, assessments, etc.
    • Train, teach, and improve skills for all nurses. Be ready to assist starting IVs and hanging IVs
    • Check lab binder make sure labs were followed up on – Ward Clerk to ensure we have results for every req and review standing orders to make sure they are done
    • Pharmacy recommendation follow up – MD faxed, received response, if new order carried out, update care plan, appropriate assessment completed if applicable
    • RD recommendation follow up – MD faxed, receive response, if MD agrees then orders carried out, update care plan if appropriate.
    • Investigations on falls – ensure Qrtly update is complete, incident report is complete, care plans updated and linked
    • Psychotropic recommendation follow up – MD faxed, received response, if MD agrees then orders carried out, update care plan, assessment, and consents if applicable.
    • Check I & O 3 day patterning Bowel & Bladder and follow up
    • Check stations for paper and cleanliness – Ward clerk
    • Check DC/expired Narcotics for D.O.N.
    • Emergency help to Charge Nurses
    • 2 PM follow up with Charge nurses for help to prevent overtime
    • Check New admission chart & IDT team notes
    • Check Alert Charting, Change of Condition, etc
    • Follow up standing orders for monthly laboratory
    • Attend weight variance, Skin, and Fall committee meeting weekly and follow up with MD, RP, etc
    • Cover stations while nurses are on break
      • Station 1 12:00pm
      • Station 2 11:00
      • Station 3 11:30
    • Will function as D.O.N. in the absence of D.O.N.
    • Follow up with room changes – Alert charting for 3 days, care plan for 3 days to monitor for patient adjusting to new environment
    • Oder Lab supplies
    • Customer Service with residents and family
    • Discharge follow up orders and supplies
    • Send back feeding pump, IV pump to pharmacy
    • Check E kits for expiration date and replacements if used
    • Check refrigerator and crash cart (CPR) – Ward clerks
    • Check utility rooms
    • Check medication carts and TX carts for cleanliness
    • New admission assessment, B&B, and Pain x 3 days
      • Fall
      • Skin x 1 week – Tx Nurse
      • Skin x 14 days – Tx Nurse
    • 602 for AL or Board and Care
    • Order medication for home with pharmacy of choice for discharges
    • Help with Discharges
    • Help with Admissions
    • Psychotropic meeting
    • Review hospital referrals
    • Recap
    • Care Conferences as needed
    • Base line Care Plans for new admits

    Qualifications

    • Current California RN license
    • Strong Assessment & Care Planning skills.
