    We currently have an opening for a Memory Care Coordinator - Assisted Living (Full Time)

    Work schedule: Sunday - Thursday (9am - 5pm)

    The Memory Care Coordinator is responsible for leading, coordinating and implementing programs and services for residents with cognitive impairment: to provide a therapeutic environment that maintains each resident’s highest level physical, social and psychological well-being.

    • Lead and maintain Memory Care Halcyon Program Model.
    • In conjunction with Assisted Living team responsible for all aspects of care, support and services for residents.
    • Know, understand, train and lead team with company program vision and values.
    • Daily champion in support of resident qualify of life-families and staff as well.
    • Work with AL Director in hiring staff and feedback during performance reviews.
    • Present concepts and facilitate discussions of policy issues.
    • Identify both individual and group activity programs for SRC.
    • Conduct pre move in assessments and interview with resident and their family.
    • Review and maintain regulatory compliance.
    • Review and communicate all new policies and other publications.
    • Actively participates in the day to day experience in the center. This is a position with emphasis on hands on leadership. At times involved in the direct care or an activity program.
    • Other duties as assigned.      


    • 2 years professional experience in Memory Care/Assisted Living required.
    • High school diploma, college preferred.
    • One year experience in conducting group activities preferred.
    • Experience in Microsoft office (other general business systems, hospitality, internet, etc.)
