    The Personal Assistant/Certified Nurse Aide for our Home Health service provides care giver companionship, general support, and light routine housekeeping for clients who wish to remain at home but may need supervision and extra care.  This position will also support care giver needs in our Health Center, Assisted Living and Memory Care as needed. 

     

    Work Schedule:  PRN/On-Call.  Must be flexible to work any day of the week, including some weekend and holiday shifts as needed.

    Responsibilities

    • Performs personal care duties as per service plan.
    • Light housekeeping, keeping living quarters clean, neat, and orderly. Vacuuming, making beds, washing dishes, maintaining clean kitchen and bathroom.
    • Planning, preparing, and serving nutritious meals.
    • Read to and write for the client as needed, provide personal interest, encouragement, and concern.
    • Accompany transport of client (provided by Trinity Terrace).
    • Set up personal items for personal care.
    • Provide personal privacy. Maintain confidentiality.
    • Observe changes in physical and emotional well-being of client and alerting appropriate personnel of these changes.
    • Utilize good judgment skills in reporting to appropriate personnel if client requires assistance with medication.
    • Be knowledgeable of emergency procedures, phone numbers, physicians, etc.
    • Notify agency if the individual needs change or conditions change on an ongoing basis.
    • Keep accurate progress notes of client’s psychological and physical condition and service performed in the home as well as performance of related duties, emergencies, or noteworthy situations.
    • Allow client the opportunity of self-care and expression according to his/her capabilities.
    • Use principles of individual and environmental safety and accident prevention.
    • Report all abnormal concerns to Case Managers and/or office (i.e., potential abuse).
    • Accurately completes daily activity sheet following care plan. Accurately complete time sheet following assignment.
    • Other duties as assigned

    Qualifications

    • Must have current certification as a Certified Nurse Aide in the state of Texas
    • Must be qualified to render care as requested by the client or client’s representative per service plan
    • Must attend required in-service education and staff meetings. Must have 6 hours education pertaining to job annually (one additional hour required for caregiver’s providing medication administrations services)
    • Current CPR/First Aide certification is required
