    We have an immediate opening for a part-time Social Worker who will primarily be responsible for providing assistance to the Social Services Director.  The Social Worker assists with identifying psychosocial needs for Health Center residents which affect their well-being, mood and behavior and develop a plan to minimize or eliminate their psychosocial problems. This position will provide help dealing with a variety of mental health, emotional health and daily living problems to improve overall functioning. This position may also provide education and support to individuals and/or family members in an effort to ensure maximum functioning and overall well-being of the resident as well as participate as part of a larger care team-based approach. Maintains confidentiality and HIPPA requirements.

     

    Work Schedule:  Days.  Hours can be flexible.  No weekends or holidays.  (15-20 hrs/week)

     

    Responsibilities

    • Work with residents of the Health Center to identify any psychosocial needs which affect their well-being, mood and behavior and develop a plan to minimize or eliminate their psychosocial problems.
    • Monitor the effectiveness of psychosocial, mood and behavior treatment plans through direct contacts with residents and information received from other staff members.
    • Review quarterly charting. Assess new problems and assist in formulating necessary treatment plans, including discharge plans.
    • Consult with the Lead Activities Assistant in setting up socialization and individual activities that will meet the unique needs of the residents.
    • Participate in Resident Care Conferences and contribute pertinent psychosocial information in order to develop an adequate interdisciplinary care plan.
    • Meet with residents at the time of admission to the Health Center to discuss Resident’s Rights, Advance Directives and Medicare/non-Medicare covered services.
    • Attend conferences to increase knowledge and skills on social services and long-term care.
    • Act as resident advocate for residents in the Health Center.
    • Assist in the completion of Admissions paperwork with Residents in the Health Center.
    • Assist with one to one visits with Residents in the Health Center.
    • Serve as a member of the interdisciplinary team for the purpose of determining the needs of residents in moving from one level of care to another.
    • Serve as a member of the safety committee, quality improvement committee and the quality assurance committee.
    • Other duties as assigned by the Social Services Director.

    Qualifications

    • Requires significant knowledge of social services laws and regulations.
    • Must have working knowledge of long-term care facility operations
    • BA/BS degree in Social Work
    • Current license to practice as a Social Worker in the state of Wisconsin
